Kindergarten Roundup is scheduled for Friday, April 24, 2020 at Treynor Elementary. There will be two sessions, one from 9:15-10:15 a.m. and the other from 1:45-2:45 p.m. If you wish to attend, please fill out the information on the Kindergarten Roundup Information Form.  You will be contacted in early April with more information and the session you are scheduled to attend.

Please note, you will need to bring the following forms:

• Child's birth certificate (we will make a copy)
• Registration Form
• Home Health Form
• Home Language Survey
• Iowa Immunization Form  (specific dates are needed for the shots received, not just the year)
  • Immunization Law Informational Brochure
• Physical Examination Form (Pre-school physical will be accepted.
  • This form must include proof of lead screening
• Vision Screening Certificate
  • There is an additional vision screening information card that will be included in your packet at Roundup
• Dental Screen Certification Form
